We are excited to offer the workshop “Plan Beyond Cancer – Survivorship Care Planning” in partnership with Gilda’s Club of Madison and generously funded by the Susan G. Komen South Central Wisconsin Affiliate. This workshop will introduce the survivorship care planning process, demonstrate our companion website (PlanBeyondCancer.org) and additional resources, and give participants a chance to plan in small groups.
